Student Medical Information
All students require a Medical Information and Consent Form to be completed twice per year. This form provides general medical information and consent for first aid.
The form can be filled out online by clicking on the title above. Please complete and email to the Front Office via info@necs.act.edu.au
Students with a known medical condition or who require medication to be administered at school will need to complete a Known Medical Condition Response Plan and/or Medication Authorisation and Administration Record. Both of these forms are available from the Front Office.
